We are excited to announce that David Raines Community Health Center will be a part of the #RamFam! The mobile clinic will be able to provide well-child exams, sick  visits, vaccinations, sports physicals and more during school hours. Our clinic will be supported by a Nurse Practioner, RN, Licensed Clinical Social Worker and Patient Care Coordinator. There is NO out-of-pocket expense for services. To participate, please download and complete an enrollment/consent form. No students will be seen without signed consent. You may also pick up forms from the school.
